Access to Care Survey
This survey is being run by the AHC Health Equity Team. It will help us understand how patients like to engage with health services, preferred ways of communicating and any barriers patients face when trying to access primary health care at AHC.
This is a short survey for any AHC patients and takes 3-4 minutes to complete. You can access the survey here.
Wellbeing Team Feedback
This survey is being run by the Whanganui Regional Health Network. It is focused on patient experience with the new wellbeing team at Aramoho.
This is a very short survey for patients who have visited the wellbeing team. The survey takes 1-2 minutes to complete and you can access it here.
Careplus Survey
To understand more about our CarePlus patients’ needs, we are asking people to complete a brief survey on what services they currently use, what they value most in their health care, and what additional services might be helpful for CarePlus.
This is a medium-length survey for patients enrolled on CarePlus. It takes 5-10 minutes to complete and you can access it here.
AHC Patient Experience Snapshot
This survey is designed to give us quick information on patients experience of their last interaction (consultation, phone call, repeat prescription request, screening test etc.) with the health centre. There are only two questions to be answered!
This is a very quick survey for any AHC patients. It’ll take less than a minute to answer and can be accessed here.

Patient Experience Survey
This is a national survey run by the Health Quality and Safety Commission designed to find out what patients’ experience in primary health care is like and how their overall care is managed between their general practice, diagnostic services, specialists and/or hospital staff. Building Expansion
AHC has purchased the former butcher’s premises beside our “post office building” and we are planning an extension to our facility. If you have any ideas on what is most important to you in regard to the physical space/design of the health centre, we would love to hear them.
